  1.                          GENERAL HEALTH HISTORY

  3. Any medications being taken, either prescribed or over the counter (OTC).
  4.  
  5. Some medications of particular concern:
  6.     
  7.   Acetylsalicylic acid (aspirin) may be absorbed in a hydrogel lens and cause 
  8.         corneal irritation (rare).
  9.     Accutane (isotretinoin) for severe acne can cause a rather severe dry eye with
  10.          resultant lens and corneal epithelial problems and blepharoconjunctivitis.
  11.     Antihistamines -cause dry eye problems
  12.     Anxiolytics (i.e. Librium, Valium)can cause dry eye symptoms and reduce blink 
  13.         amplitude.
  14.     Anticholinergics (atrophine, scopolamine) reduce tear secretion
  15.     Beta-adrenergic blocking agents (atenolol, metoprolol, oxyprenolol, pindolol,  
  16.     and possibly others) can cause dry eye and reduce tear lysozyme. 
  17.     Diuretics (such as Dyazide and Lasix) can cause decreased tearing,  
  18.     conjunctival allergic reactions and photophobia.
  19.     Rifampin (orange), tetracycline (dark brown to black), phenazopyridine 
  20.         (orange), phenolphthalein (pink, yellow) and nitrofurantoin (orange) all 
  21.         discolor hydrogel lenses.
  22.     Oral contraceptives -in some cases may cause dryness and/or edema
